मैं, ______________________ (अभिभावक का नाम), यह स्वीकार करता/करती हूँ कि:
I, ______________________ (Parent's Name), acknowledge that:

1. मैंने अपने बच्चे को भ्रमण से संबंधित सभी निर्देश और सुरक्षा नियमों का पालन करने के लिए कहा है।
   I have instructed my child to follow all instructions and safety rules related to the tour.
2. मैं समझता/समझती हूँ कि भ्रमण में कुछ शारीरिक गतिविधियाँ और यात्रा शामिल है जिसमें सामान्य जोखिम हो सकते हैं।
   I understand that the tour involves some physical activities and travel which may carry normal risks.
3. विद्यालय द्वारा उचित सुरक्षा उपायों के बावजूद, यदि कोई दुर्घटना या चोट होती है, तो मैं विद्यालय, इसके प्रबंधन, शिक्षकों या कर्मचारियों को जिम्मेदार नहीं ठहराऊँगा/ठहराऊँगी।
   Despite reasonable safety measures by the school, if any accident or injury occurs, I will not hold the school, its management, teachers, or staff responsible.
4. मैं अपने बच्चे के व्यवहार के लिए जिम्मेदार हूँ। यदि मेरा बच्चा अनुशासनहीनता या गंभीर दुर्व्यवहार करता है, तो विद्यालय को उसे भ्रमण से वापस भेजने का अधिकार है, जिसका खर्च मैं वहन करूँगा/करूँगी।
   I am responsible for my child's behavior. If my child engages in indiscipline or serious misconduct, the school has the right to send them back from the tour, the cost of which I will bear.
5. यदि मेरे बच्चे को किसी चिकित्सीय आपातकाल का सामना करना पड़ता है और मुझसे संपर्क नहीं हो पाता है, तो मैं विद्यालय प्रशासन को तत्काल चिकित्सीय उपचार करवाने के लिए अधिकृत करता/करती हूँ।
   If my child faces a medical emergency and I cannot be contacted, I authorize the school administration to arrange immediate medical treatment.
6. मैं यह घोषणा करता/करती हूँ कि मेरे बच्चे को कोई दीर्घकालिक, पुरानी, या अंतर्निहित बीमारी नहीं है जो उसे इस भ्रमण के लिए अस्वस्थ या अयोग्य बनाती हो। मैं पूर्ण रूप से समझता/समझती हूँ कि इस घोषणा पत्र पर हस्ताक्षर करना यह प्रमाणित करता है कि मेरा बच्चा शारीरिक और मानसिक रूप से भ्रमण के लिए पूर्णतः स्वस्थ और सक्षम है।
   I hereby declare that my child does not suffer from any chronic, long-term, or underlying medical condition, disease, or ailment that would render him/her unfit, unsuitable, or incapable of participating in this educational tour. I fully understand and acknowledge that signing this declaration certifies that my child is physically and mentally fit, healthy, and capable of undertaking this tour.
   
   
   यदि मेरे बच्चे को कोई गंभीर चिकित्सीय स्थिति, दीर्घकालिक रोग, या ऐसी कोई भी बीमारी है जो यात्रा या शारीरिक गतिविधियों से प्रभावित हो सकती है या बढ़ सकती है, तो S. Fatima Public School दृढ़तापूर्व्यक अनुशंसा करता है कि मैं अपने बच्चे को इस भ्रमण पर न भेजूँ।
   If my child has any serious medical condition, chronic illness, or any disease that may be affected by or aggravated during travel or physical activities, S. Fatima Public School strongly recommends that I refrain from sending my ward on this tour.
   
   
   इस घोषणा में किसी भी तथ्य को छिपाना या गलत जानकारी देना विद्यालय को उत्तरदायित्व से मुक्त करता है और मैं इसके सभी परिणामों के लिए स्वयं जिम्मेदार रहूँगा/रहूँगी।
   Any concealment or misrepresentation of facts in this declaration shall absolve the school of all liability, and I shall be solely and wholly responsible for all consequences thereof.
7. मैं स्वीकार करता/करती हूँ कि मेरे बच्चे भ्रमण के दौरान अपने साथ लाए गए सभी व्यक्तिगत सामान जैसे मोबाइल फोन, इलेक्ट्रॉनिक उपकरण, आभूषण, नकदी, या अन्य मूल्यवान वस्तुओं के लिए स्वयं जिम्मेदार होगा/होगी। किसी भी व्यक्तिगत सामान की क्षति, हानि, या चोरी के लिए S. Fatima Public School, इसके प्रबंधन, शिक्षक, या कर्मचारी किसी भी प्रकार से उत्तरदायी नहीं होंगे। मैं अपने बच्चे को आवश्यक और न्यूनतम सामान लाने के लिए निर्देशित करूँगा/करूँगी।
   I acknowledge that my child shall be solely responsible for all personal belongings brought during the tour including mobile phones, electronic devices, jewelry, cash, or other valuables. S. Fatima Public School, its management, teachers, or staff shall not be liable in any manner whatsoever for any damage, loss, or theft of personal belongings. I shall instruct my child to bring only necessary and minimal belongings.
8. मैं समझता/समझती हूँ कि यह एक एक दिवसीय शैक्षिक भ्रमण है और प्राकृतिक आपदा, दुर्घटना, यातायात विलंब, या अन्य अप्रत्याशित परिस्थितियों के कारण वापसी समय में देरी हो सकती है। ऐसी किसी भी परिस्थिति के लिए विद्यालय जिम्मेदार नहीं होगा। मैं विद्यालय द्वारा दिए गए निर्देशों का पालन करूँगा/करूँगी और धैर्यपूर्व्यक प्रतीक्षा करूँगा/करूँगी।
   I understand that this is a one-day educational tour and the return time may be delayed due to natural calamities, accidents, traffic delays, or other unforeseen circumstances. The school shall not be held responsible for any such circumstances. I shall follow the instructions given by the school and wait patiently.
9. मैं पूर्ण रूप से सहमत हूँ कि भ्रमण के दौरान या उससे संबंधित किसी भी प्रकार की दुर्घटना, चोट, बीमारी, संपत्ति की हानि, या किसी अन्य अप्रिय घटना के लिए S. Fatima Public School, इसका प्रबंधन, संचालक मंडल, शिक्षक, कर्मचारी, या विद्यालय से जुड़े किसी भी व्यक्ति को किसी भी प्रकार से उत्तरदायी, दोषी, या जिम्मेदार नहीं ठहराया जाएगा। मैं इस भ्रमण से संबंधित सभी जोखिम स्वेच्छा से स्वीकार करता/करती हूँ।
   I fully agree that S. Fatima Public School, its management, board of directors, teachers, staff, or any person associated with the school shall not be held liable, guilty, or responsible in any manner whatsoever for any accident, injury, illness, loss of property, or any other untoward incident during or related to the tour. I voluntarily accept all risks associated with this tour.
10. मैं भ्रमण शुल्क की नीति को समझता/समझती हूँ और स्वीकार करता/करती हूँ।
    I understand and accept the tour fee policy.

मैं इस दस्तावेज़ को ध्यान से पढ़ चुका/चुकी हूँ और स्वेच्छा से हस्ताक्षर कर रहा/रही हूँ।
I have read this document carefully and am signing it voluntarily.
